Please download this form, complete and bring with you to your first appointment. We are a private fee for service facility and do NOT accept any insurance. We can provide superbills for you to file with your insurance company for reimbursement.
Please note that your first appointment with Dr. Sams will be a 2-hour time slot for her to provide quality care. We value your time and appreciate if you would value ours as well. If you need to cancel your appointment, please call the office at least 24 hours before to inform us. If you get the voicemail please leave a message so we will know to cancel your appointment. Cancellations/rescheduling, no show/no call not within 24 hours will incur a fee of $85.
